DESCRIPTION:Please join us at one of our Adult Family Care (AFC) Information Sessions to learn more about this potential opportunity for both you and your loved one!\n\n \n\nWednesday\, January 11th at 10:00 am\n\nThe Arc of GHN's Haverhill Office\n\n57 Wingate Street\, Suite 301\n\nHaverhill\, MA 01832\n\n\n\nBenefits of joining the AFC Program:\n\n	RN's and Case Managers offer ongoing education\, advocacy\, and healthcare oversight\;\n	Non-taxable monetary support according to the needs of the individual cared for\;\n	 24-hour emergency line\;\n	Assistance with accessing community supports\;\n	Bi-lingual (Spanish/Eng.) Case Management is available.\n\nIn order to qualify for AFC services\, the member:\n\n\n	Must have MassHealth\;\n	Be 16 years or older\;\n	Have one or more medical diagnoses\;\n	Require at least supervision with one Activity of Daily Living (ADL)\, including: bathing\, dressing\, eating\, transferring\, or ambulating\; and\n	Cannot be left alone in the home setting for more than three hours per day.\n\n\n\nTo register or to request more information please contact Jennifer Couturier\n\nJennifer.Couturier@TheArcofGHN.org\n\n(978) 373-0552\, x204
