Fallon Community Health Plan (FCHP) is a not-for-profit health insurer that serves the health care needs of 185,000 members throughout Massachusetts. Founded in 1977, the health plan offers a range of products, including HMO plans, Medicare + Choice plans and a national preferred provider organization (PPO) plan. Based in Worcester, Mass., FCHP has an accreditation status of ''Excellent'' for its commercial and Medicare + Choice (senior) plans from the National Committee for Quality Assurance and earned a spot on their list of the top 10 health plans in the country for clinical excellence in 2003.
